Write percentage and CGPA the way your university issues it. If your marksheet says CGPA, write CGPA and add the percentage in brackets only if your college publishes an official conversion formula.
Personal details and the declaration: an Indian CV convention
Global CV advice tells you to delete the personal details block and the declaration, and that advice is written for a hiring market that is not yours.
